[WARC] Emergency contact list

I am starting to compile an emergency contact list.  This will be used for 2 
more purposes:

1.  Actual printed list for hams and emergency workers to use.
2.  The initial automatic phone list with for the auto dialer that the city 
has.

I think I need the following:

name:
callsign:
home phone:
work phone:
cell phone:
Priority preference:      1___________  2______________ 3___________   ie. 
1.cell 2 home 3 Not
email:                                                         will be 
printable but not clickable.


If you have an idea what else might be useful, please let me know.
You decide if you are going to provide the information.  Any information not 
provided will be left blank.   This is strictly an opt-in list and will not
